Registration Form – If you prefer to register for classes by mail or fax, please use this form. It includes the Health Clearance Form (below).

Health Clearance Form – Mandatory for anyone taking a "movement" class (any class under the movement/exercise category or the yoga category).

Medical Clearance Form – If you have answered "yes" to any of the questions on the Health Clearance Form, the Medical Clearance Form must be completed by your primary care clinician and returned to the office before you can participate in a movement/exercise or yoga class.

Acupuncture Registration Form – Mandatory for clients who would like to schedule an acupuncture appointment and who have never had this type of appointment at the CWHC before.

Massage Registration Form – Mandatory for clients who would like to schedule a half-hour or one-hour massage therapy or Reiki appointment and who have never had this type of appointment at the CWHC before.

On-Site Chair Massage Guidelines – Informational document for clients who want to set up on-site chair massage at the location of their choice.

Shiatsu Registration Form – Mandatory for clients who would like to schedule a Shiatsu appointment and who have never had this type of appointment at the CWHC before.

CHI Application Form – Form for undergraduate students interested in applying to be a Community Health Initiative (CHI) representative.
