Registration Form – If you prefer to register for classes by mail or fax, please use this form.

Exercise Class Participant Agreement – Mandatory for non-Harvard affiliates (includes spouses, alumni, dependants, and retirees) who would like to enroll in a movement class.

Acupuncture Registration Form – Mandatory for clients who would like to schedule an acupuncture appointment and who have never had this type of appointment at the CWHC before.

Massage Registration Form – Mandatory for clients who would like to schedule a half-hour or one-hour massage therapy or Reiki appointment and who have never had this type of appointment at the CWHC before.
